If an Officer has requested you complete a witness statement, there are a few ways you can do this.
1. From a tablet or smart phone using Adobe Fill and Sign (See instructions below)
2. Handwrite on a statement you obtain from BPD or print out yourself
3. Type on a computer or in a word processing application and write "See Attached" on the witness statement. All documents must contain your signature.
*For options 2 and 3, you may scan and email the documents, bring them in person, or send photos (least preferred) to the Officer handling your case.
Adobe Fill & Sign
iOS (Apple Devices)
Download the Adobe Fill & Sign application from the app store.
Open the witness statement on your device by clicking the link.
Tap the share button (Square with an arrow pointing up) then choose "Save To Files" at the bottom of the options.
Open Adobe Fill & Sign and tap "Select a Form to Fill Out" then choose "File Browser"
The recently saved witness statement should be the first available file.
Select the witness statement and it will open within the application.
Tap the ? for a tutorial on how to use the app to complete the form.
Tap the share button (Square with an arrow pointing up) when you are done, and email the statement to the Officer handling your case.

Download the Adobe Fill and Sign application from the app store
Download the statement from the link
Open Adobe Fill and Sign
Tap the graphic at the top with the plus sign and the paper
Select “From PDF File”
Then select the Downloads folder, and you should see the witness statement file to select.
Follow the instructions on Adobe Fill and Sign. You can tap the ? to view a tutorial on how to use the app at any time. Email the statement back to the Officer handling your case.
