Record Request by Mail/Email
The records division is open Monday through Friday 8:00 a.m. to 5:00 p.m. and closed holidays.
If you would like to submit via mail, please mail your completed record request form to: 805 South Main Street, Bountiful, Utah 84010

Requests will be processed as quickly as possible. In most cases citizens will be able to obtain a copy of their report in one visit to the records division. However, in some cases it may take ten business days or longer to receive a copy of the report depending upon the circumstances of the investigation. A records personnel will keep you updated of the status of your request.
Please remember, you must pick up the report in person and show photo identification.
