Record Request by Fax
The records division is open Monday through Friday 8:00 a.m. to 5:00 p.m. and closed holidays.

Record Requests can be submited via online, mail or email, or by fax.
If you would like to submit via fax, please fax your completed record request form to 801-298-6027
Requests will be processed as quickly as possible. In most cases citizens will be able to obtain a copy of their report in one visit to the records division. However, in some cases it may take ten business days or longer to receive a copy of the report depending upon the circumstances of the investigation. A records personnel will keep you updated of the status of your request.
Please remember, you must pick up the report in person and show photo identification.
